Clinic FormsIf you would like your child to be considered for treatment at our clnics, here are the required forms in PDF - please click to download: Please read: Read and mail in these forms: Getting into clinical servicesOur clinics are faced with a very high demand for services. The best time to enter into clinic services is at the start of each of our two treatment semesters. Our school year semester runs from late August to early June, our summer semester (Social Thinking Stevens Creek) runs from mid June to late August. We usually fill all available treatment slots prior to the start of each semester and start a wait list at that time. We fill spots from the wait list as treatment slots become available, however students are contacted from the wait list based on how they best match the open treatment slot. Thus, we do not go sequentially through the waitlist to fill our open slots. Parents are also required to send us a written application at the start of each semester to indicate their availability for treatment sessions for each upcoming semester. If a parent does not send in a new application for each semester, we will assume the family is no longer pursuing treatment through our center. A deposit of $210 is required once your child is placed into services. The deposit will be held and applied to the last two scheduled sessions.
